If more than one name appears under a number this means that they acted as a Dungeon Master Team. That is the only way a Dungeon Master should be listed more than once on a list.
Comments: Great first game for the campaign. You've kept the module design clean and simple - a good idea for a purely tabletop style, DM'd adventure.
Since you're going with that style (which is a good choice), I'd suggest concentrating on building good NPC's, items, and areas as we go along. In particular, you might try adding some description texts on important NPC's and placeables/items - but perhaps that takes up too much prep time.
